CHR/Healthy Living Program
The Community Health Representative (CHR) is an advocate and facilitator for individuals and families to gain access to comprehensive healthcare services. The goal is to provide a continuum of services to the community through health education case finding, referral, follow-up, and provision of support services. The CHR is often the first vital link to assure access to the comprehensive health care system. The CHR serves as a facilitator in the implementation of community, family, and individual health and treatment plans and provides supporting services designated to enhance the quality of life for all eligible American Indians.
Healthy Living Exercise Program: Consists of low-impact, power-assisted exercise chairs to high-impact exercise machines. Provided a healthy and comfortable place to work out and learn alternative ways of eating healthy and taking care of one’s health to promote a healthier living style.
Taking vital signs or providing other clinical services, such as foot care, to persons with a diagnosed illness. Also included are services such as:
Home health care and maintenance of equipment such as wheelchairs, walkers, shower chairs, canes, and other equipment
Health Education /Health fairs
Case find/screening
Case management
Monitor patient
Connect transportation (Pelivan)
Delivery of medication, supplies, and equipment to patients’ home
Events- for the program coalition to gather to socialize, celebrate and support each other. This includes, but is not limited to holiday and birthday parties, Elder’s Day, health and exercise classes, and foot care.
